Abstract     We know from the literature and our own experience that in patients with chronic renal failure bone changes develop at a very early stage of renal disease. We are presenting the case of SR, a female patient born in 1938, who deviated strongly from the average. She was first admitted to the Department of Nephrology in 1985. At that time we made the diagnosis of chronic pyelonephritis. In 1994 she began complaining of bone pain, limited joint mobility, she was only able to walk on crutches. The diagnosis was secondary hyperparathyroidism. Th.: calcitriol in daily doses of 0.25-0.50mg and CaC03 3x1 g/day. In 1996 the patient started hemodialysis treatment. Bone biopsy confirmed the diagnosis of mixed osteodystrophy. The patient was dialyzed 3x weekly for 4 hours on a Gambro AK 100 device with a Gambro Plus 16 dialyzer. The average established Kt/V was over 1.2. The patient received calcitriol in doses of 2mg after 3x weekly dialysis treatment until October 1997, then the dose was lowered to 1 mg/3x weekly. In April 1998 calcitriol treatment was discontinued.
