Abstract     With aging of the population, heart failure is becoming a greater social, professional and economic problem. Among many reasons for heart failure, arrhythmias have a special place. Because of controversial effects of antiarrhythmics, it is very important to find out an answer to a question on which patients to treat and which drug to use? In the case of acute heart failure, we often decide on the electroconversion. In chronic heart failure, drugs are prefered, especially amiodarone, beta-blockers, and ACE-inhibitors. In therapy, digitalis still has its place as well. In the case of bradycardias, electro stimulation is very important. In patients with atrial fibrillation and uncontrolled ventricular response, we prefer radiofrequency catheter ablation in combination with permanent pacemaker implantation.
