Abstract     In a prospective study conducted from 1 January 1995 to 31 October 1996, 96 patients with endoscopic and histological evidence of gastric cancer were evaluated by endoscopic ultrasonography (EUS). The aim of the study was to assess the accuracy of EUS in the preoperative TN staging of the tumor. The results were compared with the definitive pathohistological stage determined after operation. In eight patients who underwent only exploratory laparotomies, EUS results were compared with intraoperative findings. The overall accuracy of EUS in assessing the T stage was 77,08%. The highest accuracy was achieved in T4 (91,7%) and T 1 tumours (87,5 %), and the lowest in T2 lesions (60%). The accuracy of EUS in assessing the T stage was not significantly affected by the histological type of the tumour, its localization and macroscopic type. The causes of misdiagnosis were related to objective limitations of EUS in detecting microscopic tumour invasion and the ability to distinguish between peritumours inflammatory changes and tumours tissue. The N stage was correctly assessed in 64/96 patients, the overall accuracy being 67%. The sensitivity of the investigation was 74% and its specifity was 53%. EUS is a highly accurate method for the evaluation of gastric cancer and a useful tool in selecting patients for endoscopic treatment of early cancer and preoperative chemotherapy.
