Abstract | Serious infections caused by resistant microorganisms are very common in patients with dysfunctional immune system. The management of these patients requires admittance to an intensive care unit, which is an ideal environment for the emergence of antibiotic resistance. Problem pathogens presently include multiply resistant gram-negative bacilli, methicillin-resistant Staphylococcus aureus and vancomycin resistant enterococci. Initial treatment is usually empirical and good knowledge of the pharmacodynamics of antibiotics is very important. Inadequate antimicrobial treatment results in high mortality and raises the cost of hospitalization. Epidemiological surveillance, control and appropriate antibiotic-use policies are cornerstones for prevention of antimicrobial resistance. | |
Summary | Bolniki z zmanjšano telesno obrambo pogosto zbolevajo s hudimi okužbami, ki jih nemalokrat povzročajo bakterije, odporne proti različnim antibiotikom. Zdravljenje teh bolnikov pogosto poteka v enotah za intenzivno zdravljenje, kjer je število odpornih bakterij največje. Največjo težavo povzročajo po Gramu negativni bacili, ki so odporni proti različnim antibiotikom, Staphylococcus aureus, odporen proti meticilinu in enterokoki, odporni proti vankomicinu. Ker je zdravljenje običajno izkustveno, je nujno dobro poznavanje farmakodinamike antibiotikov. Neustrezna izbira antibiotika povečuje smrtnost in ceno zdravljenja. Stalni epidemiološki nadzor in ukrepi ter kontrola predpisovanja antibiotikov so najpomembnejši dejavniki, ki zavirajo razvoj odpornosti proti antibiotikom. | |
