Abstract     In blood transfusion, compatibility between the donor and recipient of all blood cells has to be taken into account due to existence of polymorphic allogeneic traits, which are expressed in all types of blood cells. The immune response to foreign antigens after transfusion or in the pregnancy leads to various forms of hypersensitivity to erythrocyte, thrombocyte and granulocyte antigens, which have to be diagnosed and treated accordingly. For the diagnosis and corresponding treatment, both serological and molecular genetic investigations that enable DNA-based antigen typing are available; their usefulness is most evident when we are dealing with the problems regarding fetomaternal sensitisation.
Summary     Zaradi alogenskih polimorfnih dednih znamenj, ki se nahajajo na vseh krvnih celicah, je treba pri transfuziji krvi upoštevati skladnost med dajalcem in prejemnikom. Imunski odziv na tuje antigene lahko po transfuziji in v nosečnosti povzroči nastanek različnih oblik preobčutljivosti na eritrocitne, trombocitne in granulocitne antigene, ki jih moramo ustrezno diagnosticirati in zdraviti. Za oboje imamo na voljo serološke in v zadnjem času tudi molekularnobiološke preiskave, ki omogočajo tipiziranje antigenov na ravni DNA, njihova uporabnost pa je najbolj očitna pri problemih v zvezi s fetomaternalno senzibilizacijo.
