Abstract     Reliable diagnosis of malaria requires laboratory confirmation of the presence of malaria parasites in the blood of the patient. Even if we are entering in the 21st century, the laboratory techniques of the 19th century remain in routine use and continue to be the gold standard: microscopic examination of the Giemsa stained thick blood film for diagnosing malaria (and other blood parasites) and the Pappenheim stained thin blood film for the detection of the characteristic stages of plasmodium parasites in the peripheral blood and the determination of their quantity and species. The detection a small percentage of parasitised cells requres a high quality microscope and experienced microscopist. A new technology is available nowadays for low level infections: the Quantitative Buffy Coat method based on the hematocrit principle, the fluorochrome acridine orange dye for DNA and RNA of the plasmodium and fluorescent microscopy.
Summary     Zanesljiva diagnoza malarije zahteva laboratorijsko potrditev, da so v krvi preiskovanca prisotni malarijski paraziti. Čeprav vstopamo v 21. stoletje, ostaja še vedno v rabi kot zlati stan- dard laboratorijska tehnologija 19. stoletja: mikroskopski pregled po Giemsi obarvane goste (debele) kaplje za dokaz prisotnosti malarijskih ali drugih krvnih parazitov ter po Pappenheimu obarvanega krvnega razmaza za določitev razvojnih stopenj, količine in vrste plazmodijskega parazita. Dokaz majhnega deleža s paraziti okuženih eritrocitov zahteva kakovosten mikroskop in izkušenega mikroskopista. Lažje jih dokažemo s koncentracijo parazitov na način, podoben določanju hematokrita, ob prisotnosti akridin oranžnega barvila in njegove fluorokromne privlačnosti do DNA in RNA v parazitu in s pomočjo fluorescentne mikroskopije.
