Abstract     Transformation of epithelial hyperplasia (EH) of the larynx and hypopharynx to squamous carcinoma (SC) are accompanied by stromal changes. The aim of this study was to analyze the diagnostic and prognostic significance of stromal myofibroblasts, lymphocytic infiltration, and extracellular matrix proteins tenascin (T) and fibronectin (FN) in EH and SC of the larynx and hypopharynx. The expression of T and FN gradually increased with the degree of EH and was markedly increased, in SC. Myofibroblasts were found exclusively in SC. We observed two patterns of stromal changes in SC: one was characterised by marked desmoplasia with proliferation of myofibroblasts, abundant deposition of T and FN, and scarce lymphocytic infiltration: This pattern was associated with well or moderately differentiated SC. The other pattern was characterised by weak desmoplasia with few myofibroblasts; little T and FN, and dense lymphocytic infiltration. This pattern was associated with moderately or poorly differentiated SC. We conclude that proliferation of myofibroblasts is the only stromal change that is limited to SC and could be therefore used as a stromal marker of malignancy: Our results show that the characteristies of stromal changes in SC could be of prognostic significance.
