Abstract     Synovial sarcoma is rare malignant tumor usually found on the extremities, but very rarely in the head and neck region. About 110 cases of head and neck synovial sarcomas have been reported in the literature. Synovial sarcomas arise from mesenchymal and not from synovial tissue, and are mostly manifested as biphasic tumors consisting of epithelial and spindle cells. Monophasic epithelial and spindle eell pattern may lead to diagnostic confusion, which may be resolved by immunohistochemistry techniques. Early diagnosis is established on the basis of the symptoms such as pain, dysphagia, hoarseness or tumor mass inducing cosmetic deformity. Most of the patients come to medical attention because of a painless tumor mass; CT scan and MRI are helpful in determination of exact dimensions, growth extent and borders of a tumor. The choice of therapy depends on histopathology and site of the tumor. The therapy includes both the extensive surgical removals of the tumor and postoperative radiotherapy. The authors report about their two cases of synovial sarcoma. The first one is atypical and totally different from any other case presented in the literature with its dimensions, age of the patient, absence of local and distance metastases. The second case had characteristics that were similar to the most of described cases. CT or MR imaging was done through diagnostic process of both cases. Final diagnosis was established by pathologist after surgery.
