Abstract     Background: The aim of study was to determine if serum estradiol (E2) dynamics in periovulatory phase can be used to predict the outcome of IVF in natural cycle and if this parameter is related to other already known factors that influence outcome of IVF in natural cycles. Methods: 234 natural IVF cycles were analysed within this prospective study. ANOVA was used to compare serum EZ levels on each day and serum E2 ratio of two succesive days in the periovulatory phase (day -3 until day + 2; day 0= day of HCG injection) among the groups with a different outcome. Ez levels and ratios in which difference was observed were included into a logistic regression model and their connection to premature ovulation, positive aspiration of follicles, fertilization of the ovum, implantation of the embryo and pregnancy was studied. The most appropriate cut-off value using a ROC curve was determined to the factor which was related to the pregnancy. Based on this value the cycles were divided into two groups and compared according to the women's age, cause of infertility, basal values of LH and FSH, duration of the cycle, E2 values, diameter of the follicle, thickness of the endometrium on day 0 and quality and speed of the division of the embryo. Results: The difference within the changes of E2 levels in separate outcomes of cycles were observed between the days -1 and +1. During this time the changes in E2 were related to premature ovulation and implation of the embryo. Only the connection between days 0 and +1 was related to pregnancy (B= -1.750, p= 0.001), the most appropriate value for assuming pregnancy was 1.1. The groups divided according to this cut-off value differ only in the value of E2 on day 0 and not in the other factors. Conclusion: On the basis of the serum levels of EZ between the days -1 and +1 the possibility of a premature ovulation or implantation of the embryo can be predicted.
