Abstract     Different possibilities for the origin of iatrogenic perforations of the esophagus, their localisation, methods of operative treatment and prognosis with regard to the localisation and duration of the perforation are described. The description is accompanied by a presentation of author's own cases. The use of laser for recanalization of the esophagus in malignant obstruction is mentioned as a new etiologic factor for the origin of perforation. The radical access was used successfully in a inveterated case of laser perforation of the esophagus, whereby the septic focus was removed in the first phase, so that the part of the esophagus containing tumor and perforation was excised through a thoracotomy and the remaining stumps were sutured bindly. Five days later a gastrostomy and a collar esophagostomy were done. Two months after that, the digestive tract was reconstructed by an esophagogastrostomy in the neck.
