Abstract     Complications of iatrogenic penetrations produce high mortality rate in particular if they are recognized early but no immediate treatment is introduced. Penetrations of the lower part of oesophagus go on to more serious complications than penetrations of the upper part in the cervical region. The basic illness plays an important role in the development of complications as well as time lapse from penetration until diagnosis has been made, patients age and patients general condition. Direct complications are due to inflammatury causes. Following penetration the infection can spread in the mediastinum and pleural cavity because of anatomic position of Oesophagus. Trickling of stomach contents through the petrating oesophagus produce virulent and necrotizing infections and maintains septicemia. Mediastinal, pleural and pericardial exudate may influence the cardiopulmonary functions. The aim of surgical treatment is to achieve a good drainage and to hinder further contamination. The infection must be treated with suitable antibiotics and the failure of multi organ failure mus begin with specific treatment. The patients are fed with parenteral fluids or nourished directly into stomach or small intestinum (jejunum). We present three patients of whom one suffered from penetration following orotracheal intubation. In the other two penetration followed laser treatment for narrow Oesophagus due to Cancer growith. All the patients developed complications, i.e. infection, sepsis and failure of various body organs which were in direct connection to penetrating injury of Oesophagus.
