Abstract     Twelve patients (pts) with 6 spontaneous and 6 iatrogenic cholesterol embolizations (ChE) observed in the period 1990-93, are presented. ChE was histologically confirmed in 8 autopsies (S% of 160 consecutive autopsies in persons over 60 years of age), in 3 kidney biopsies or resections, and by fundoscopy in 1 pt. Spontaneous ChE was found in 4 autopsies, but in another 4 it developed after instrumental procedures on great arteries, including 1 pt with a kidney transplant. There were 3 (0.29%a) pts with ChE among 1,048 kidney biopsies and resections (including the material from transplanted kidneys). ChE occurred spontaneously in 1 pt, after percutaneous transluminal angioplasty in the second, and in the third, ChE has recurred in the second renal allograft. In 4 of 6 pts with spontaneous ChE, but only in 1 of 6 pts with iatrogenic ChE, multiple organs were affected. The incidence of ChE is nowadays mostly iatrogenic in origin, and is probably higher than it is actually diagnosed. ChE has to be considered in every patient with a deteriorating renal function. The diagnosis of ChE in 2 consecutive renal transplants (representing 0.7% of 282 transplant biopsies and transplanectomies) suggests that the probability of Che and its recurrence in the renal allograft recipients is substantial.
