Abstract     Vegetative dysfunction clinically established in Parkinson's disease is described in the introduction. Results of pathomorphological and biochemical research demonstrating lesions not also inside, but also outside the nigrostriatal system as well, and modified function of the dopaminergic and other transmitter system are described. Published results of clinical research of disturbed cardiovascular regulation in Parkinson's disease are often contradictory and only a few studies have dealt with factors which influence the cardiovascular function. We tried to detect and measure the impairment of cardiovascular reflexes in Parkinson's disease patients and how they are by a variety of factors. 50 patients with Parkinson's disease, aged 42 to 70 years (mean 59.1 years) and 30 healthy subjects, aged 50 to 70 years (mean 58.0 years) were studied. Five cardiovascular reflexes were performed by all subjects: the Valsalva manoeuvre, the deep breathing test, the face immersion test, the handgrip test and the orthostatic test. We added spectral analysis of heart rate variability to the testing battery as well. Results of measurements in healthy subjects and in patients and specifically results of testing with regard to the duration, severity and type of the disease, age and gender, subjective orthostatic troubles and treatment were compared. In patients significantly altered reactivity of blood pressure and heart rate, i.e., signs of dysfunction of both parasympathetic and sympathetic nervous systems, were detected by all tests except for the face immersion test. The autonomic dysfunction differed among the studied patient subgroups. Blood pressure reactivity decreased already in the early years of the disease and in less severely ill patients, while heart rate reactivity was found to be impaired in more than one test only in patients with a disease of longer duration or a more severe disease.(trunc.)
