Title     Plasminogen-activator-inhibitor type 1 and failure of thrombolysis with streptokinase of patients with acute myocardial infarction

Abstract     Failure of thrombolysis with streptokinase (SK) may be due to antifibrinolytic effect of platelets derived type 1 plasminogen-activator-inhibitor (PAI-1). Therefore, in 98 patients with acute myocardial infarction (AMI), treated with STK, the relationship between failed thrombolysis and pre-treatment PAI-1 levels, determined by chromogenic method (normal range 0,3-3,5 U/mL), was studied. Thrombolysis with STK failed, when reperfusion was delayed, as assessed by noninvasive reperfusion criteria, or reinfarctions developed. The average pretreatment PAI-1 level was 6,1 +- 15,0 U/mL, range 0,3-66,2 U/mL. Thrombolysis with STK failed significantly in patients with elevated pre-treatment PAI-1 levels (p<0,05), mainly due to reinfarction (p<0,05), but less to delayed reperfusion (p>0,05). We conclude: failure of thrombolysis with STK was significantly associated with elevated pre-treatment PAI-1 levels.
