Title     Cardiocirculatory failure in the course of multiple organ dysfunction syndrome (early recognition and management)

Abstract     In patients with multiple organ dysfunction syndrome (MODS), the symptoms and signs of cardiac failure can be the consequence of pre-existing cardiac disease or the pathophysiological response to the systemic inflammatory response syndrome (SIRS). The degree of circulatory failure depends on the degree of hypovolemia, cardiovascular action of various inflammatory mediators and previous cardiac diseases. Beside respiratory changes, the response of the heart and circulation is one of the earliest clinical manifestations of SIRS or MODS. Therefore, these changes must be recognized as soon as possible in the course of the MODS to allow early and correct treatment. Initial evaluation of cardiac function should and could be made according to patients history, clinical investigation, ECG, chest X-ray and results of blood gas analysis. However, in the majority of patients some special noninvasive (echocardiography) and invasive (right heart catheterization) examinations have to be performed to assess the cardiac function properly. Once the basic parameters of cardiac function are estimated, the treatment should be started. Beside volume therapy and use of very potent vasoactive drugs, early mechanical ventilation, IABP insertion and continuous hemofiltration could be very effective and even lifesaving procedures in patients with refractory heart failure in the course of MODS.
