Abstract		The treatment of Tossy or Rockwood III-rd grade acromioclavicular dislocation remains controversial. There is always a reasonable doubt whether the primarily operative approach is really justified. It is claimed the results of the conservative treatment equals the operative without the risks and complications of the operation itself. We compare our long term operative results wifh the literature. Our patients have more complicafions with the remaining dislocation or with the greater physical demands. We believe more complications are to be expected with the remaining dislocation, so we recommend primarily operative treatment for the patients with the higher expectations.
