Abstract     Tendons of muscles supraspinatus, infraspinatus, subscapularis and teres minor compose fhe rotafor cuff. They arise from scapula and insert on humerus at the lesser and greater tubercle. They cover the head of humerus as a cap and fix it in the glenoid in the beginning of the movement, so that the deltoid muscle can Jift the arm over the horizontal plane. All these muscles act as internal or external rotators or depressors of the humerus. After the 40th year of age degeneration occurs due to special anatomical situation at the insertion of the tendons. Rotator cuff can then rupture with minimal injury or sometimes even without it. Acute tears in younger persons wifhout previous degeneration are rare and occur in high-energy frauma. Usually the abruption of the insertion of the tendons occurs in such cases. In younger persons with acute injury immediate surgery is indicated, the unsuccessful functional treatment is the indication for the operation in older persons.
