Abstract     Backgrounds: The rotator cuff tear is the most frequent pathological modificafion in the shoulder joint. Even though the majority of injuries is the result of degenerative modifications, the acute injury can likewise cause the rotator cuff tears. For acute injuries in younger injured persons the majority of authors recommend the earliest convenient operative treatment. The injured persons selection and time to the final treatment are significant. methods and resu;ts. In Celje General and Teaching Hospital (CGTH) an algorithm to detect injured persons with acute rotator cuff tears was introduced. In 4-months period from November 2000 to March 2001 the traumatologists in CGTH treated 11 injured persons with rotator cuff tears. By detecting acute injuries the data on the injury mechanism was of key importance (positive prospective value 0.78). UZ shoulder examination became a standard examination to confirm the clinical rotator cuff diagnosis, and to establish the rupture size. Seven injured persons were operated in less than 2 months after the injury (the average time from the injury to the operative intervention was 8 weeks). The massive rotator cuff degenerative rupture was found in two patients. On the basis of first experiences we modified criteria after Loewe for acute rotator cuff tears. To follow up the treatment results we have developed a protocol, which makes the detailed prospectively collected data analysis feasible. Conclusions: By including a traumatologist into diagnostics and operative treatment of the rotator cuff tear we can significantly decrease time to the final treatment. By the prospective analysis of results we can establish the efficiency of such an approach to the rotator cuff tear treatment.
