Abstract     Background. Rotator cuff tears are frequently treated surgicaly. By the present retrospective analysis it was attempted to give the insight into the results of full-thicknes cuff surgery with an average follow-up of four years. Methods. Fifty-one patients (37 men and 14 women), treated for rotator cuff rupture at the Department for Orthopaedic Surgery and Sports Trauma of the Celje General Hospital from July 7992 to December 1996, presented for the control check-up. The shoulder function in these patients, aged 53 years (mean, range 25 to 77 years) was assessed according to the Constant Classification. The strength of abduction and of external rotation was additionaly measured on the apparatus for medical training therapy. The opposite shoulders of the same patients were used as controls. Results. The mean value of the Constant score for the operated shoulder was 82,7 (range 26 to 700), and for the non-operated shoulder 92,4 (mean, range 37 to 100). The satisfactory score (Constant at least 60 points) was found in fourty-five patients (88,2%). The result of treatment was significantly correlated to the size of the tear (p=0,0037) and the time elapsed from injury to surgery (r = -0,3969; p=0,02). The satisfactory score (Constant at least 60 poinfs) was found in fourty-five patients (88,2%. Measurements on apparatus for medical training therapy showed reduced power in treated shoulders compared to the peer control shoulders (p<0,05). Conclusions. After so-called watertight repair of cuff tears and anterior acromioplasty, successful results can be anticipated in a high percentage of patients, especially if they are treated early after injury. However, no perfect strength improvement could be anticipated after reconstruction of larger cuff tears.
