Abstract		Issues. Division between chronic, spontaneous and acute posttraumatic changes of the rotafor cuff represents and demanding task for the medical assessor. lnadequate biomechanical analysis and estimation of shoulder trauma complexity leads to wrong decisions in therapy and in medical assessment. N Methods. An overview of recent expert literature with an emphasis to analysis of traumatic event, biomechanics of injury and objectification of the extent of acute trauma of the rotator cuff increases quality of assessment in Slovenia. Objectification of traumatic event, injury extent documentafion, clinical picture follow-up, determination of all possible causes and extent of chronic changes of rotator cuff vitally influences on assessment at determining acute injuries of the rotator cuff. Conclusions. Consideration of new biomechanical and anatomical discoveries of strains posed to each part of rotator cuff has also changed views in assessment procedures. Previous citations of possible mechanism of injuries of rotator cuff have become inappropriate and are superseded with new findings. Change of perfusion of parts of rotator cuff especially supraspinatus, role of micro trauma, clinically silent changes until trigger moment, guide the assessor at his work. Finding of rotator cuff tear by ifself does not explain cause, time and mechanism of the mentioned change.
