Abstract     Background. Efficiency of perioperative intraperitoneal application of Mitomycin-C bound to activated carbon particles (MC-CH) in patients with advanced gastric carcinoma was evaluated. Patients and methods. 35 gastric cancer patients (T3/4, Nx,M0) who underwent standard R0, D2/3 resection are included in a prospective randomized multicentric study, 17 patients are randomized for perioperative intraperitoneal MC-CH application and 18 ones for the control group. Results. Postoperative mortality 8,5% (3/35), one patient in the MC-CH group and two in the control group. In the MC-CH group 4 patients developed an intraabdominal abscess and one patient an enterocutaneus fistula. In the control group one patient had anastomic leakage and one patient laparotomy rupture. 5-year survival-calculated for 32 patients with pT3/4 tumor-is 17,1%. There was no significant difference in long term survival between MC-CH and control group (p=0,37184). Conclusion. Adjuvant intraperitoneal therapy of advanced gastric cancer with MC-CH is associated with an increased rate of postoperative complications, without significant benefit in long-term survival.
