Abstract | Background. One of the branches in the development of the endoprostheses is the custom-made bone implant. In contrast with the classic implants in which the surgeon is forced to adjust the bone to the implant with custom-made technology, the implant is adjusted to the morphology of the bone. Methods, results. Here it is also presented an attempt of the unification of the technologies of computer tomography, computer aided design (CAD) and computer aided manufacturing (CAM) which resulted in the design and manufacture of a custom-made hip implant for a cadaveric proximal femur and a stereolithographic model of the same bone. Conclusions. The custom-made implants did not improve the curvivorship of the prostheses as expected. Today the use is limited mostly to though cases of very displastic joints or bad posttraumatic cases and situations after extensive resections due to bone neoplasms. In the other fields of secondary joint reconstructions modular implants, which are increasingly user friendly, lead the way. | |
Summary | Izhodišča. Z razvojem endoprotetike se je razvila tudi tehnologija izdelave protez po meri bolnika. Za razliko od klasične endoprotetike, kjer kostnino prilagodimo vsadku, gre tu za individualno izdelavo vsadkov po kostnem modelu. Metode in rezultati. Prikazan je primer združevanja tehnologij računalniške tomografije, računalniško podprtega oblikovanja in tehnologije hitre izdelave prototipov po meri z izdelavo endoproteze kolka po meri kadavrske proksimalne stegnenice. Zaključki. Tehnologija izdelave protez po meri ni prinesla izboljšanja trajnosti kostnih vsadkov, zato se danes ohranja predvsem pri rekonstrukciji sklepnih displazij, zapletenih posttravmatskih stanjih ter stanjih po obsežnih resekcijah kostnih neoplazm. Na ostalih področjih sekundarne endoprotetike prevladujejo modularni, kirurgu prijazni in enostavni vsadki. | |
