Abstract     A patient with ischemic chest pain at rest or minimal exertion in duration of more than 5-10 minutes, which is not relieved by nitroglycerine, has probably acute coronary syndrome. Acute coronary syndrome includes unstable angina pectoris, acute myocardial infarction and sudden ischemic cardiac death due to ventricular tachycardia/fibrillation. The present review describes pathophysiology, patomorphology and clinical features of acute coronary syndrome. Current approach and treatment, which begins in the prehospital setting and continues in the emergency department and coronary care unit in the hospital is discussed. Authors suggest more unified approach to the acute coronary syndromes in Slovenia and also provide a vision for the future of this field of medicine.
