Abstract     The life of a patient with massive hemoptysis is in danger, prompt treatment is mandatory. Mortality is a function of the amount of blood, underlying disease/condition and correct treatment. The cause of death, as a rule, is not exsanguination but asphyxia due to the blood flooding the bronchial system. The site as well as the cause of hemoptysis has to be determined; a chest radiogram is often enough, at times computerised tomography (CT) and/or bronchoscopy are necessary. Bronchoscopy is the first choice in an unstable patient with massive hemoptysis, in a stable patient CT is first used. The first choice of treatment used to be surgery; this has been largely replaced by bronchial artery embolisation, which is less aggressive, with lower mortality. The aim of treatment is preservation of vital function, hemostasis, dealing with the underlying cause of bleeding.
