Abstract     Purpose: A nonrandomized comparative study was undertaken to determine wheter an original modified technique of interposition vein patch improved the long term results of femoropopliteal and femorocrural PTFE bypass procedures. Patient and methods: The comparative study between 3 group of patients was done. The first group represented 88 patients undergone femoropopliteal or crural bypass procedures in the period between March 1992 and Februry 1999 in our institution. PTFE with interposition vein patch was employed as bypass graft. The second group were 191 patients operated in the same period using autogenous saphenous vein grafts.The third group were our previous series of 94 patients operated in the period between January 1985 and February 1992 using PTFE grafts without interposition vein patches.The indication for bypass procedure was critical lower leg ishemia in all groups. The follow up period ranged from 6 to 66 months. Primary, secondary patency and limb salvage rates were calculated using life table method. The statistical significanse was assesed with log rank test. Results: The 5 year primary,secondary patency and limb salvage rates for PTFE grafts with vein patches were 53,4 +- 7,4 %, 60.9, +- 7,7 %, 68 +- 6.6 % for below knee popliteal grafts and 30,5 +- 7.3 %, 38 +- 8.5 %, 40,2 +- 8.3 % for infrapopliteal bypass grafts, respectively. The 5 year primary,secondary patency and limb salvage rates for saphenous vein grafts were 68,9 +- 4 %, 74,9 +- 3.7 %, 86,6 +- 4.8 % for below knee popliteal grafts and 51,4 +- 7.3 %, 55,2 +- 7 %, 59,5 +- 7.3 % for infrapopliteal bypass grafts, respectively. The 5 year primary,secondary patency and limb salvage rates for PTFE grafts without vein patches were 29,7 +- 6,4 %, 37,8 +- 6,8 %, 43,6 +- 6,7 % for below knee popliteal grafts and 6,9 +- 5.1 %, 11,3 +- 8,3 %, 14.6 +- 6.3% % for infrapopliteal bypass grafts, respectively.
