Abstract | Study objective: To evaluate reproductive results in different grades of uterine anomalies before and after transcervical discisions of uterine septa by retroscope. Study design: Retrospective analysis of 232 metroplasties by resectoscope. Setting: Reproductive unit, University woen hospital, Ljubljana, Slovenia. Interventions: Hysteroscopic metroplasties were performed between 1993 and 1996. The 5% dextrose was a distending medium and the automated gravitational Vario flow system was used for uterine distention. Main results: The structures of 338 pregnancies before the operation significantly differed to the structure from 78 to 11% (p<0.0001). The delivery rate increased from 13 to 81%. Preterm deliveries dropped from 50 to 10% of all deliveries (p<0.0001). The ectopic pregnancy rate showed no difference: 8 and 7%. Perinatal and neonatal data in 100 deliveries after metroplasty showed no difference compared to the data in 200 deliveries in normal controls. There were 40% of small septa (AFS 6) and 60% of more important septa - 1.5 cm - total septum (AFS 5). The spontaneous abortion rate per all pregnancies dropped from 81 to 10% after 94 discisions of AFS grade 5 uterine septa (p<0.0001). The preterm delivery rate dropped from 55 to 2% (p<0.0001) of all deliveries after metroplasties for minor anomalies (AFS 6), and from 43 to 13% after discision of AFS grade 5 uterine septa (p<0.002). Conclusions: The results highlight the curative and indicate the possible preventive role of this safe and simple endoscopic procedure. | |
