Abstract     Between March 1991 and December 1993, 64 patients with inoperable head and neck carcinoma were randomly assigned to receive either radiation therapy alone (group A) or radiotherapy combined with stimultaneous application of Mitomycin C and Bleomycin (group B). Disease free survival (DFS) at four years was in B group patients 37%, and in group A 8%(p=0.01). DFS for patients with oropharyngeal carcinoma only was in B group 48% and in group A 10% (p=0.001). Since December 1993, 95 consecutive patients with advanced inoperable oropharyngeal carcinoma were routinely treated with above scheme. DFS (55%) was comparable to that of the randomized group B patients. Based on previous favorable experience, the same combination was tested as postoperative treatment in selected group of patients with greater probability for recurrence due to unfavorable prognostic factors. Preliminary results of 57 evaluable patients included in the study indicate that the effect of postoperative radio-chemotherapy is better than that of postoperative radiotherapy alone. In the group of patients treated by radiotherapy alone, the disease recurred in 10/27 patients (37%), while in the concomitant group the recurrence was observed in 5/30 patients (17%).
