Abstract     Clonidine in brachial plexus block prolongs analgesia of local anesthetic of short and intermediate duration. We performed a prospective randomized study to frnd out, if clonidine mixed with long-acting local anesthetic ropivacaine does effect the onset time of sensory block, its quality and duration. Forty-six patients, who underwent surgical procedure of upper arm bone fractures, were enrolled and randomly divided into two groups. We used interscalene brachial plexus block and injected 20-46 ml of 0,'75% ropivacaine i control group (n=23), and 20-46 ml of 0,75% ropivacairie mixed with clonidine (1microg/kg of body weight) in clonidine group (n=23). We recorded the onset time of sensory block, motor block quality, latent time, block duration, side effects and quality of pain relief on a visual analog scale. The onset time of sensory block (10,9 vs 11,7 min.), motor block quality (22/1/0 vs 19/4/0 and latent time (32,6 vs 31,6 min.) were not statistically different between control and clonidine group šp > 0,05). Hemodynamic variables (sistolic, mean, diastolic arterial pressure and heart rate) were statistical insignificant (p > 0,05) between the groups. The clonidine mixed to 0,75% ropivacaine did not statistically significantly prolonged the duration of block (743 vs 764 min:) or effected the pain relief on a visual analog scale (1,9 vs 1,65). We conclude that clonidine in a dose 1 microg/kg of body weight does not effect the onset time, quality and duration of interscalene brachial plexus ,block with 0,75% ropivacaine. Ropivacaine alone ensures perfect operative conditions and good postoperative analgesia.
