Abstract     Background.ln this case report a 51 years old female patient is represented with progredient polyneuropathy and osteoporosis haveing bad regulated diabetes for 33 years and used to smoke very intensively for 25 years. After hip neck fracture of left femure and progredient form of osteoporosis the surgeon decided to treat the patient conservatively therapy with extension imobilisation in bed. After being imobilised for 47 days in bed- extensive calcanic area ulcers developed with consecutive periostitis and osteomyelitis of both calcaneus bones. Diabetes wasn't possible to regulate any more. Electrolyte disturbances progressed and the patients husband ( M,D.) decided for hospital treatment of her wife. The epidural catheter was introduced to perform epidural analgesia and sympathicolysis - to make possible the ulcers to heal and to enable operative treatment of the bone fracutre. After mobilisation of the patient she left hospital with healed ulcers and osteoosintesis of the left femure after 134 days of hospitalisation. (Abstract truncated at 2000 characters)
