Abstract     Background: Food rich in cereal fiber, especially wheat fiber has been shown to protect against aterosclerotic cardiovascular disease. Patients with the metabolic cardiovascular syndrome (MCVS), which includes insulin resistance, dyslipidemia, arterial hypertension and increased blood thrombogenic potential, are at risk for accelerated atherosclerosis. The effects of wheat fiber supplementation on cardiovascular risk factors in these patients are not known. Subjects and methods: Forty-two persons with MCVS, 31 males and 11 females, aged 27 to 65 years (mean age 49+-8 years), were randomly assigned to control and treated group. Patients in treated group consumed 10,5 g of wheat fiber powder supplement for the first week and then 21 g for four weeks in addition to their regular diet. Patients in control group maintained their regular diet only. Anthropometric measurements were performed and blood samples were collected at the entry of the study, after five weeks and after four weeks of washout period. We measured blood glucose, insulin, lipids, plasminogen activator inhibitor (PAI) antigen and activity, tissue type plasminogen activator (t-PA) antigen and activity, plasminogen and factor VII activity and fibrinogen. At the entry and after five weeks treatment period oral glucose tolerance test (OGTT) was performed. Results: Nineteen patients in treated group and 18 patients in control group completed the study. After five weeks of treatment period patients in both groups lost on average one kg of body weight. Diastolic blood pressure decreased only in treated group (for an average of 7 mm Hg, p=0,03), while systolic blood pressure decreased in both groups, more in treated group.
