Abstract     Background: Due to early detection, breast carcinoma is increasingly diagnosed in its early stage. Despite the fact that prognosis is relatively good in patients with T1N0M0 tumors and their 10-year survival is about 80%, most of these patients will, regard to the current guidelines, receive systemic adjuvant treatment. There is no general agreement as to which clinical or pathologic parameters are of prognostic significance in this group of patients. Apart from tumor size, histologic grade, histologic type and vascular invasion have been shown to be prognostically important in several studies. The prognostic value of new biologic markers has only rarely been studied in T1N0M0 breast carcinoma. Aims: The aim of the study was to assess morphologic features and incidence, extent and pattern of the expression of bcl-2, p53, c-erb-B2, carcinoembryonic antigen (CEA), Ki67 (MIB-1), estrogen (ER) and progesterone receptors (PR) in T1N0M0 breast carcinoma and to evaluate their relation to patient survival. It was hypothesized that a combination of new biologic markers and classical prognostic factors could identify different risk groups and thus help in treatment decisions. Material and methods: The study included 270 patients with primary T1N0M0 breast carcinoma who underwent surgery at the Institute of Oncology between 1983 and 1987 and who were followed for a median of 12.5 years. All original slides were reviewed and examined for histologic type, mitotic index (MI), extent of tubule formation, pleomorphism, Nottingham histologic grade (NHG) and presence of vascular invasion. Representative tumor slides were stained immunohistochemically for p53, bcl-2, c-erB-2, MIB-1 (Ki67), CEA, ER and PR using LSAB method and Dako TechMate 500 automatic immunostainer. The prognostic value of investigated features was evaluated using univariate and multivariate survival analysis. (Abstract trunacted at 2000 characters).
