Abstract     Prognostic factors pertinent to treatment modalities were studied on accumulated 237 cases of stage IA cervical cancer treated during 1981 to 1997 in our Department. Patients with early invasive cervical cancer who have a minimal risk of extracervical spread and can be treated with less radical surgery were identified by using a scoring system of morphological parameters. In the first period (198188), 36.1 % of cases were treated by conization, 33.7% by simpleand 30.2% by radical hysterectomy. In the most recent observation period (1993-1997), conization was performed in 65% of the cases, simple hysterectomy in 27.5%, hysterectomy with pelvic lymphadenectomy in 7.5%. There were no cases with indications for radical hysterectomy. Our experience indicates that conservative management of stage IA cervical cancer is indicated and safe when exact evaluation of tumor extension and surgical margins of the cone are assured.
