Abstract     Backgrouod: To prevent premature rise of the luteinizing hormone (LH), gonadotropin-releasing hormone agonists have been used over the last decade in in vitro fertilization procedures. Since 1999 the premature LH peak has also been prevented by the first two representatives of GnRH antagonists, cetrorelix and ganirelix. The aim of this prospective randomized study was to compare the efficacy of GnRH antagonists vs. GnRH agonists in in vitro fertilization procedure.Methods: In the study group we enrolled 42 randomly selected infertile patients in whom induction of ovulation was performed using the combination of human menopausal gonadotropins (HMG) and GnRH antagonists according to the single dose protocol. In the control group we enrolled 32 randomly selected patients, in whom ovulation induction was performed using the combination of HMG and GnRH agonists according to the conventional protocol. The inclusion criteria were the upper age limit of 38 years, normal body weight, tubal factor infertility and endometriosis as the causes of infertility, regular menstrual cycle and normal spermiogram of the partner .Results: In the study group the duration of ovulation induction was significantly shorter (median 8 days) than in the control group (median 10.5 days). Also, the number of ampoules of HMG used was significantly lower in the study (median 20) than in the control group (median 31). The pregnancy rate per embryo transfer (26.5% in the studt and 36.7% in the control group) and the mean implantation rate (14.7% in the study and 26.7% in the control group) were lower in the study group, although not significantly. On oocyte retrieval the amount of inhibin B in the follicular fluid of the dominant follicle was significantly greater in the study (median 97525 than in the control group (median 59759).
