Abstract     The acute coronary syndrome includes a whole spectrum of clinical diagnoses ranging from unstable angina pectoris and various forms of myocardial infarction to a sudden ischemic cardiac death. A quick and correct intenrention before hospitalisation and a quick admission to hospital are highly important. In patients with the ST elevation, we immediately attempt a recanalisation of the thrombosed coronary artery either with thrombolytic therapy or primary angioplasty. All other patients are submitted to treatment with antiaggregation, anticoagulation and antianginal therapy. Patients older than 70 and those with angina occurring at rest and with ST depression, especially in cases of increased troponin values, are considered as high-risk cases. The prognosis for such patients can be improved by invasive approach (early coronary angiography and angioplasty) and by administering platelet Ilb/Illa antagonists.
