Abstract | Buerger's disease (BD) is a segmental occlusive vascular disease, which primarily affects small distal arteries, mostly in young male adults. The disease is closely related to cigarette smoke exposure and it goes on with exacerbations, while other risk factors for atherosclerosis are usually absent at the time of setting the diagnosis. It is not quite clear yet, whether the disease alterations of vessel wall are consequences of inflammation. The aim of the present study was to find out if there are any functional disorders and asymptomatic morphological changes at the time of the disease remission on peripheral arteries, where the disease did not express clinically. At the same time we assessed the presence of laboratory signs of inflammation and endothelial damage. We wanted to test the hypothesis, that BD is principally systemic inflammatory process, which is also present at the time of the disease remission and accompanied by diminished functional capability of the peipheral arteries. Subjects and methods. 14 patients with BD (10 male, 4 female) and the same number of age and sex matched healthy volunteers were included in our study. By measuring haemodynamic changes in brachial artery during the provocation tests (reactive hyperaemia, sublingual administration of nitroglycerin) we echographically assessed the capability of endothelium- dependent and endotheliumindependent dilation of brachial artery. We also echographically investigated the structure of the wall of carotid arteries and measured intima-media thickness (IMT). Besides basic haematological and biochemical examinations we determined blood fibrinolytic activity and blood concentration of some cell adhesion molecules (ICAM-l, VCAM-l, E-selectin) and TNF-alpha.
