Abstract     Acute infective endocarditis (IE) is one of the most serious complications of bacteremia and/or sepsis. The treatment should be prompt and includes intensive medical treatment and early valve surgery. The mortality rate of acute IE is usually greater than 30 % and depends on the pathogen, the extent of valvular damage and on the systemic complications. The most common pathogens are gram positive cocci, namely Staphylococcus aureus, Streptococcus pneumoniae, Streptococcus pyogenes , and, rarely, Gram negative bacteria and fungi. Echocardiography is a most important diagnostic method for detection of IE. The isolation of pathogen from blood cultures is decisive for successful antibiotic treatment. Early cardiac surgery is often the only treatment alternative especially in a patient with refractory heart failure and local intracardiac complications.
