Abstract     The article deals with a 34 years old patient with acute coronary syndrome with ST elevation in ECG derivations D2 and D3. Troponine T was negative, CK enzyme 3,20 and MB 4,36. Coronary angiography revealed dissected left main stem and proximal left anterior descending artery. The dissection was demonstrated angiographically and the off pump aorto-coronary venous bypass graft operation was carried out (LAD and D1) instead of stenting or thrombolytic therapy. The patient was discharged 8 days after operation. The following consequences could be drawn from the reported case report: not all coronary artery occlusions are caused by a thrombus and atherosclerotic plaque; it is not possible to predict the nature of coronary stenosis without at least coronary angiography; spontaneously dissected coronary arteries are most frequently occluded and not subtotally narrowed as was the case in our patient. In a described case, thrombolytic therapy may have had a hazardous and harmful side effect.
