Abstract     Introduction. Mallory-Weiss tear is a common source of upper digestive tract bleeding. Case report. Presented is the case of a 54-year old male patient, who was admitted due to upper gastrointestinal hemorrhage and hemorrhagic shock in intensive care unit (ICU). During interventional endoscopy, a 12x4 mm large tear, Mallory-Weiss syndrom, at the gastroesophageal junction was found. Endoscopic injection hemostasis was performed, leading only to a transient success. The patient died two hours later due to exanguination. At autopsy a giant tear measuring 55x5 mm was found, with a large vessel stump, probably the left gastric artery. Conclusions. Acute upper gastrointestinal hemorrhage is frequently of unpredictable course.
