Abstract     Article presents problems that occurs when prehospital emergency services provide care for morbidly obese patients. In USA, each year dies more than 300.000 patients because of their overweight related problems. Almost every day our service, which provide EMS coverage for Ljubljana, take care of one or more obese patients. Overweight by itself is not a problem. The problems are overweight related complications like: high blood pressure, coronary diseases, heart failure, diabetes etc.. Patient assessment, management and transportation of obese patients are unique. Special problem present patient transfer from old high-rise buildings without elevators. Emergency care at obese patients is another problem (airway management, IV lines etc..). Authors present regular equipment for patient handling and also present special equipment for overweight patients transportation and other rescue maneuvers. In our case it is very important that all services that provide patient care and works together for patient during rescue operations are familiar with all procedures and equipment. University Medical Center Ljubljana Ambulance Service and Ljubljana Fire Brigade are good example of proper obese patients emergency care.
