Abstract     The outcome of resuscitation depends on primary cause of cardiac arrest, efforts of bystanders to perform cardiac pulmonary resuscitation (CPR), the arrival time of emergency medical service (EMS) and its skills, early defibrillation. If we suppose to have very well trained EMS, we can influence the outcome of resuscitations by encouraging and lead bystanders to perform CPR. We report a case of successful out of hospital CPR, when the dispatcher has simultaneously alarmed EMS and led bystanders to performing CPR. The importance of communication between bystanders and dispatcher is discussed. Early CPR improves survival in out-of-hospital cardiac arrest, and dispatcher-delivered instruction in CPR can increase the proportion of arrest victims who received bystander CPR before EMS arrival. The dispatcher should be the most experienced in the field of emergency medicine and communication.
