Abstract     The change in the balance between Thl and Th2 (Thl/Th2) cytokines is an important factor in the pathogenesis of autoimmune diseases. There are contradictory data about this change in Graves' disease (GD). Antithyroid drugs such as methimaaole (MMI) are the first choice for the patients with GD and presumably have some direct immunomodulatory action. Aims: The aim of the study was to determine Thl/Th2 cytokines in the patients with newly diagnosed GD, and compare the data to the group of healthy controls. The shift in Thl/Th2 cytokines was also evaluated in the patients with GD after one year of MMI treatment, when compared to the same balance in patients with newly diagnosed GD before treatment and in healthy controls. Methods: Peripheral blood mononuclear cells (PBMC) were isolated from 17 healthy volunteers, from 18 patients with newly diagnosed GD before treatment and from 15 patients with GD after one year of MMI treatment, who were clinically euthyroid and had normal values of TSH, fT4, fT3 and TSAb. The PBMC were activated with ionomycin or CD3 antibodies in combination with phorbol 12-myristate 13-acetate (PMA). After 40-hour incubation, the culture supernatants were collected and concentrations of Thl/Th2 related cytokines (IFN-gamma, IL-12 and IL-4, IL-10) were measured by ELISA (Endogen, USA). The t-test was used to compare the concentrations of cytokines produced and the ratios of Thl against Th2 cytokines between the three groups studied. Results: After activation with ionomycin and PMA PBMC from patients with GD before treatment produced significantly less IL-12 and significantly more IL-10 and IL-4 than PBMC from healthy controls.
