Abstract     Inflammatory bowel disease (IBD) is a current gastroenterology problem because of its increasing prevalence and incidence in the developed world and because of the number of unexplained questions. IBD is a group of chronic inflammatory disease of unknown aetiology that affects gastrointestinal tract. The whole IBD group is characteristic for its great heterogeneity (phenotypic, genotypic) and chronic, usually life long and unpredictable course with recurrent acute and chronic inflammation of intestinal segments. According to their phenotypic and genotypic characteristics they are divided into two main groups: ulcerative colitis (UC) and Chron's disease (CD). UC and CD are related diseases but nevertheless there are significant differences and characteristics in the course, development and extent as well as in treatment and prognosis of the disease. That is why a clear difinition and separate treatment of the disease is important. Defining IBD and separating it from the other GIT diseases, as well as defining its subgroups UC and CD, according to the symptoms and clinical signs, which are very unspecific, is usually very difficult. To make the accurate diagnosis it is necessary to carry out further examination, which is very invasive (endoscopy with biopsy, contrast radiological examination with barium). Despite adequate clinical evaluation and the help of invasive diagnostic tools the percentage of cases, which cannot be clearly defined, is relatively high (8-20%). Aetiology of IBD is still unexplained. There is involvement of genetic factors (HLA and nonHLA genes) and environmental factors (infectious diseases, smoking, and nourishment). In recent years IBD has been connected to the immune system disorder and the phenomenon of autoimmunity. (Abstract truncated at 2000 characters).
