Abstract     Delivering bad news is one of the most difficult challenges for pliysicians and other members of a health care professionals' team. Patient's future relationship with doctor and patient's psychophysical reaction about illness depend on how the news is delivered to the patient. Around the world skills of delivering bad news are taught mainly according to the Canadian model SPIKES, as a flexible step by step protocol which consists of following steps: Setting, Perception of the patient, Invitation, Knowledge, Emotions, Strategy and Summary. Suggested protocol is especially valid for skills required for the step of emotional support to the patient which is supposed to be the most difficult part of delivering bad news to the patient. There is an evident need to evaluate the application of protocol in Slovenia, to adapt it to our specific needs and to include it into our medical curriculum.
Summary     Sporočanje slabe novice je eden najtežjih izzivov za zdravnika in tudi za druge člane zdravstvenega tima. Od tega, kako slabo novico sporočimo, je pogosto odvisen nadaljnji odnos z bolnikom, pa tudi njegovo psihofizično odzivanje v bolezni. Po svetu poučujejo veščine sporočanja slabe novice večinoma po kanadskem modelu SPIKES, ki je prilagodl jiv način stopenjskega sporočanja. Vključuje: pripravo, ugotavljanje, kaj bolnik že ve, ugotavljanje, kaj bolnik želi izvedeti, sporočanje slabe novice, podpore bolnikovemu čustvenemu odzivu ter skupno načrtovanje za prihodnost s povzetkom. Predlagani model še posebej poudarja natančne in občutljive veščine za podporo čustvenim odzivom bolnika in tako predlaga strategijo za najbolj občutljivi del sporočanja slabe novice. Tudi v slovenskem prostoru bo potrebno preveriti uporabnost modela, ga prilagoditi našim razmeram in vključiti v redni izobraževalni program v zdravstvu.
