Abstract     Lyme borreliosis is a tick transmitted infectious disease, caused by Borrelia burgdorferi sensu lato. It is endemic in Slovenia with a frequency up to 155 new cases/100,000 inhabitants registered every year. It can affect numerous organs and organ systems such as skin, joints, muscles, the nervous system, heart, and eyes. An early and a late form of the disease can be distinguished, depending on the duration of symptoms. Diagnosis can be made quite easily when a clinical presentation is typical: erythema migrans, lymphocytoma, acrodermatitis chronica atrophicans, radicular pain with positive epidemiological history. However, the clinical picture is often atypical. It can mimic numerous other conditions of a completely different origin and treatment approach. In such cases, it is much harder to establish the correct diagnosis. Diagnosis of Lyme borreliosis is established upon epidemiological features, clinical presentation and laboratory findings. Serological examination is important, but interpretation of results is sometimes difficult. Results can be negative in the early course of infection. On the other hand, positive results don't necessarily prove the aetiology of the disease. Borrelial antibodies can be present even in a healthy subject resulting from a previous asymptomatic infection. The most reliable diagnostic test is a positive culture of B. burgdorferi sensu lato from tissue samples (skin, blood, cerebrospinal fluid, ežtc.), but the procedure is dižcult, expensive, available in few laboratories and successful only in a certain number of patients. Patients with Lyme borreliosis are treated with oral or parenteral antibiotics. Amoxicillin, azythromycin, cefuroxime-axetil, doxocycline, ceftriaxone and others are available. The choice depends on clinical presentation, duration of symptoms and organs affected. (Abstract truncated at 2000 characters).
