Abstract     In the first part of this chapter, the basic functional neuroanatomy of the genitourinary and anorectal systems is briefly described. These systems are involved in the socalled sacral functions (micturition, defecation, erection, ejaculation, etc.). In this section we describe clinical neurophysiological tests of the functional integrity of the sacral neuromuscular system used for diagnostic purposes: The second part of this chapter deals with intraoperative neurophysiological monitoring of the lumbosacral nervous system. The validity of different intraoperative monitoring techniques of this system is summarized.
