Abstract     A topographically oriented study was conducted to determine if the pathohistological structure of a periapical granuloma varies with the clinical symptoms. Thirty-three specimens of human periapical granuloma were obtained by extraction or endodontic surgery. Each tooth was assigned to one of the following groups: symptomatic - asymptomatic, endodontically treated untreated. Immunohistochemically labelled sections (CD 68) were used to calculate the areal density of macrophages in the inner, middle, and outer thirds of the lesion. The overall areal density of macrophages was also determined. The results of this study confirm our hypothesis that during an acute exacerbation, the highest areal density of macrophages in a periapical granuloma occurs in the outer third of the lesion, and so progression of the process in humans is mainly due to macrophage activity.
