Abstract     Early fixation of fractures contributes to lower morbidity and mortality in patients. Intramedullary fixation with locking nails with (minimal) reaming is currently biomechanically superior to all other operative procedures for the fixation of long bone fractures. The present report adresses several controversial issues related to intramedullary fixation such as the patient's position and type of table, pri- ority of fixation, effect of intramedullary reaming in polytraumatized patients and in patients with specific injuries and conversion of external to internal fixation.
