Abstract     The increasing interest in measuring homocysteine has led to the development of new methods for determination. Determination of homocysteine is performed mostly on plasma. Concentration of homocysteine in collected blood samples increases at ambient temperature. The increase is inhibited at low temperature and by inhibitors: citrate, EDTA and 3-deazaadenosine. The latter causes interference in immunoassays. Use of fluoride is not recommended because of its osmotic effects. Homocysteine is most frequently determined by HPLC method with fluorescence detection. The method requires derivatization in addition to reduction of homocysteine. Laboratories without HPLC equipment commonly use immunoassays. The first automated immunoassay was based on detection of fluorescence polarization (FPIA). Some ELISA assays were also developed. Enzymatic methods with colorimetric detection could possibly be adapted to most of biochemical analyzers. However, these methods are still not developed for routine use. Choice of appropriate method for the determination of homocysteine should be based on anticipated number of samples and predicted application of the results.
Summary     Naraščajoče zanimanje za določanje homocisteina je spodbudilo razvoj novih metod za določanje. Najpogostejše je določanje v plazmi. V odvzetem vzorcu krvi koncentracija homocisteina pri sobni temperaturi narašča. Naraščanje poleg nizke temperature zavirajo citrat, EDTA in tudi 3-azaadenozin, ki pa povzroča inerference pri imunokemijskih metodah. Uporaba fluorida zaradi osmotskih učinkov ni priporočljiva. Za določanje se največ uporablja HPLC metoda s fluorescenčno detekcijo, kjer je potrebna poleg redukcije še derivatizacija homocisteina. V laboratorijih brez HPLC aparature so pogosta izbira imunokemijske metode. Prva avtomatizirana imunokemijska metoda temelji na detekciji s fluorescenčno polarizacijo (FPIA), razvite so tudi ELISA metode. Najboljše možnosti za prilagoditev večini biokemijskih analizatorjev ponujajo encimske metode s kolorimetrično detekcijo, ki pa še niso razvite za rutinsko uporabo. Izbira najbolj primerne metode za določanje homocisteina je odvisna od pričakovanega števila vzorcev in namena določanja.
