Abstract     Background: Coronary artery disease (CAD) is a local manifestation of advanced atherosclerotic process. It is insidious in onset and affects simultaneously coronary, carotid and peripheral conduit arteries. CAD is a multifactorial disease which appears to be due to a number of genetic and environmental factors. Our present understanding of atherosclerosis pathogenesis suggests that genetic variations in factors modulating vasomotion (ACE, e- NOS), thrombosis (t-PA, PAI-1, GP IIIa) and lipid metabolism (apoE) may be involved in the development of CAD. Aims: 1) to assess the extent of coronary and peripheral atherosclerosis in patients showing first clinical manifestations of CAD; 2) to identify possible differences in the coronary and peripheral atherosclerotic burden with respect to clinical presentation of CAD (acute MI vs. unstable AP) and age at onset (< 55 years vs. >_ 55 years); 3) to investigate the role of several gene polymorphisms in the development of CAD; 4) to elucidate possible association between genetic polymorphisms and functional or morphological markers of atherosclerosis; and 5) to identify those genetic risk factors that might be related to the mode and time of CAD presentation. Subjects and methods: We studied 100 consecutive patients presenting with acute coronary syndrome as the first clinical manifestation of CAD. The study group was divided into subgroups according to the clinical presentation of CAD (acute MI, unstable AP) and age at onset (< 50 years, > 50 years). The control group consisted of 80 age- and gender-matched healthy subjects free of symptoms and signs of CAD. (Abstract truncated at 2000 characters).
