Abstract     The efficacy of multimodal treatment of primary gastric non-Hodgkin's lymphoma was assessed in a retrospective study of 46 patients treated from 1984-1989. The principal treatment plan consisted of surgical resection (n=35) and additional therapy which was chosen according to prognostic factors: 1) no effraction of serosa and/or no infiltration of resection margins and/or stage I; 2) effraction of serosa and/or infiltration of resection margins and/or stage II or II2. Patients with low grade of non-Hodgkin's lymphoma from the first group had no additional therapy, those from the second group were irradiated. Patients with high grade non-Hodgkin's lymphoma from the first group were treated by chemotherapy and those from the second by chemotherapy and irradiation. CHOP (cyclophosphamide, dexorubicin, vincristine, prednisone) chemotherapy regimen was mainly used. Irradiation field included upper abdomen or stomach bed, dose being 2100-3000 cGy. Patinets with inoperable disease and those who refused surgery were treated by irradiation therapy (low grade non-Hodgkin's lymphoma) or chemotherapy and irradiation (high grade non-Hodgkin's lymphoma). The median follow up time was 3 years (range 1 month to 5 years 10 monthes). The estimated 5-year survival rate for 43 evaluable patients was 91 percent, the estimated 5-year disease free survival rate was 98 percent. The above results show that primary gastric lymphoma is a highly curable disease if treated by multimodal therapy.
